The paperwork that runs your practice, signed properly.
Credentialing and reappointment applications, delegated credentialing rosters, BAAs, W-9s, employment and compensation agreements. Send them in minutes — and keep a record that stands up when someone asks, two years later, who actually signed.

Signer role
Add a field
How it works
Set it up once. Send it forever.
Place the fields on a document a single time. Every request you send from it inherits that layout, so the tenth credentialing packet takes as long as pressing send.
- Step 1
Place the fields
Upload the PDF and drag signature, initial, date, and text fields onto the page. Assign each one to a role — Physician, Medical Director, Administrator.
- Step 2
Send to real people
Name the person behind each role and choose how strongly they must prove who they are. Signing happens in order, and each person is emailed only when it reaches them.
- Step 3
Get a sealed record
When everyone has signed, the values are merged into the PDF, a certificate of completion is appended, and the whole document is cryptographically sealed.
Why it matters
Courts argue about attribution, not integrity.
Almost nobody disputes whether a document changed. They dispute whether a specific person signed it. That is the part most e-signature setups record thinly — and the part MedSign is built around.
Identity assurance you choose per document
A W-9 can go out on an email link. A compensation agreement should not. For anything a signer might later have reason to deny, require a one-time passcode sent to a mobile number you supplied — not one they typed in themselves.
Consent captured before anything is fillable
ESIGN requires affirmative consent to transact electronically, given before the signature is collected. MedSign blocks every field until the signer checks the box, and records exactly which version of the disclosure they agreed to.
An audit trail that cannot be quietly edited
Every event is appended, never updated or deleted, and each entry is hashed together with the one before it. Removing or altering a single row breaks the chain from that point forward.

Each entry is hashed together with the entry before it. This table is reproduced in full on the certificate of completion inside every signed document.
Everything a security questionnaire asks about
Hospitals, payers, and NCQA reviewers ask the same questions during procurement. MedSign is built so the answers already exist.
Tenant isolation
Every record carries an organization ID, and the database enforces separation independently of the application.
Append-only audit
Hash-chained events. Updates and deletes are rejected by the database, not merely avoided by convention.
Key custody
The document-signing key lives in a key management service. The application cannot read it.
Documented process
A written declaration of exactly how a signature is produced and how tampering is detected — the document your counsel will ask for.
